Ambassador Health of Sidney, a 46-bed skilled nursing facility, is seeking a Registered Nurse (RN) to join our leadership team as our MDS Coordinator / Assistant Director of Nursing (ADON).
No previous MDS or ADON experience? That's okay. We're willing to invest in the right person and provide comprehensive training to help an experienced nurse develop the knowledge and skills needed to succeed in the role.

About the Role
The MDS Coordinator / ADON plays an important role in both the clinical and operational success of our facility.
As MDS Coordinator, you'll oversee and coordinate the Minimum Data Set (MDS) assessment and care planning process, working closely with nursing, therapy, social services, dietary, physicians, residents, and families to help ensure each resident's needs and goals are accurately reflected in their plan of care.
As ADON, you'll work alongside and support the Director of Nursing, helping provide clinical leadership, staff education, and oversight of resident care.

Responsibilities include:
- Coordinating and completing MDS assessments accurately and timely
- Helping develop individualized, resident-centered care plans
- Coordinating with the interdisciplinary team throughout the assessment and care planning process
- Monitoring resident status and identifying significant changes in condition
- Supporting appropriate documentation and coding practices
- Participating in resident admissions, transitions, and discharge planning
- Collaborating with residents and families regarding goals of care and discharge planning
- Educating and supporting nursing staff regarding care plans, documentation, and resident care
- Working closely with nursing, therapy, social services, dietary, physicians, and other members of the interdisciplinary team
- Assisting the Director of Nursing with clinical operations and nursing leadership
- Supporting quality improvement initiatives and regulatory compliance
- Helping ensure resident care is provided in accordance with applicable federal and state requirements
What We're Looking For
You do not need previous MDS or ADON experience to be considered. We're looking for an RN with strong clinical judgment, attention to detail, communication skills, and the desire to learn.
Required:
- Current RN license or ability to obtain appropriate licensure
- Strong clinical assessment and critical-thinking skills
- Excellent organization and attention to detail
- Strong written and verbal communication skills
- Ability to collaborate effectively with residents, families, staff, and other healthcare professionals
- Interest in learning MDS, care planning, reimbursement, quality, and nursing leadership
Preferred, but not required:
- Skilled nursing or long-term care experience
- MDS or utilization review experience
- Nursing leadership experience
