Position: PT (Physical Therapist) Home Health

Job Type: Full-time or Part-time

Shift: Day Shift (Primarily Mon-Fri. Weekends as needed. Schedule is flexible)

Compensation: Compensation based on experience

Work Location: Home health travel in the Omaha area


  • Provides physician prescribed physical therapy.

  • Improves or minimizes residual physical disabilities of the patient.

  • Returns the individual to optimum and productive level within the patient's capabilities.

  • Takes initial history and makes initial evaluation.

  • Consults with physicians regarding change in treatment.

  • Instructs patients and family/significant others in home programs and activities of daily living.

  • Prepares clinical and progress notes.

  • Assists physician with evaluating level of function.

  • Helps develop the plan of care and revise as necessary.

  • Completes and submits OASIS assessments, reassessments at least every 60 days, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames an in accordance to 175 NAC 14-006.04H1.

  • Appropriately utilizes ICD-10 codes.


  • Nebraska Physical Therapy License (Required)

  • Willingness to obtain Iowa Physical Therapy License upon hire


  • Previous physical therapy experience preferred but not required

Medication Aide (40 Hour Certified)/CNA Full-time Day & Night ShiftOmaha

Position: Certified Medication Aide (40 Hour Certified)


Job Type: Full-Time

Shift: Day & Night Shift Available (6:30AM-7:00PM or 6:30PM-7:00AM)

Compensation: $17.50-$25.00 per hour $1000 hiring bonus

  • Assist in the administering of medications as ordered by the attending physician, under the direction of the attending physician, the nurse supervisor or charge nurse, and the Director of Clinical Services. 
  • The administration of medications shall be in accordance with established nursing standards, the policies, procedures, and practices of this facility, and the requirements of this state.
  • Maintain and accurately document medications as ordered by the attending physician, following facility policies and procedures
  • Observe and verify that medication is ingested or applied as directed. Document any instance where prescribed medication is not administered, including reason(s) for refusal.
  • Ensure that no medication is administered without a written, signed, dated order
  • Verify the identity of the resident before administering the medication treatment.
  • Provide application of aseptic dressings per facility policies and procedures, with the exception of treatment to a decubitus or stasis ulcer of any stage. Examples of acceptable dressing changes include application to Skin Tears and Minor Wounds/scrapes/irritations.
  • Maintenance and monitoring of oxygenation delivery systems. Fill portable oxygen tanks for resident use, when needed. Change oxygen flow from concentrator to portable and back again as needed. Adjust oxygen flow per physician’s order.


  • Must be a 40 hour Certified Medication Aide, having successfully completed a state approved training program and any necessary examination.

Job Responsibilities:

  • Monthly Skilled Nursing Facility Billing Process including charge entry, claims creation, month end reports, triple check process, reconciliation reports, claims follow up and organization
  • Daily Home Health Billing Process including claims creation, claims review, claims transmission, claims management vendor relationship
  • Skilled Nursing Facility and Home Health Accounts Receivable support processes including claims reconciliation, deposit transactions, Medicare claims status updates, Accounts Receivable AR aging reports
  • Medical Records processes, file creation and organization
  • Universal training and development to support Accounts Receivable and Billing process.
  • Other support functions such as deposit transactions, mail processing, and quarterly and annual report compilation.

Job Qualifications:

  • Strong work ethic and willingness to learn
  • Preferred comparable work experience, certifications, and/or degree
  • Microsoft Office knowledge: Outlook, Excel, and Word
  • Experience with an Electronic Health Record is helpful
  • General technical proficiency with accuracy and attention to detail

Schedule and Benefits:

  • 32-40 hours/week, generally 8am to 5 pm Monday through Friday
