Assistant Director of NursingFull-time Day ShiftNebraska City

Position: Admissions Nurse/Medical Records

Job Type: Full-Time

Shift: Day Shift (Monday through Friday)

Compensation: Based on experience

Duties:

  • Assure complete medical records program is maintained for resident, clinicians, and billing
  • Oversee MCO authorizations and weekly updates, nursing daily schedule and long term care insurance
  • Conduct comprehensive assessments of patients upon admission
  • Collaborate with healthcare team members to develop individualized care plans
  • Coordinate and facilitate the admission process, including completing necessary paperwork and obtaining patient information
  • Provide education to patients and their families regarding their condition, treatment options, and available resources
  • Assist with discharge planning and ensure appropriate follow-up care is arranged
  • Administer medications and treatments as prescribed by physicians
  • Monitor patient progress and report any changes or concerns to the healthcare team
  • Maintain accurate and up-to-date medical records

Duties:

1. Complete simple food preparation tasks.

2. Cooperate in keeping entire kitchen clean and in order at all times.

3. Keep own work area straightened and clean to aid in efficiency of work performed.

4. Report equipment repairs needed to the Dietary Manager.

5. Prevent food waste.

6. Pour juices and beverages (coffee, tea, milk, cocoa) and serve on trays at correct temperature, neatly, and in amounts called for at the proper time.

7. Fill orders for between-meal nourishment.

8. Place desserts on serving dishes and serve them on trays.

9. Prepare and serve salads, fruits, dry cereals, and cold breads on trays at proper time, attractively, and at correct temperature.

10. Wrap flatware in napkins.

11. Set up trays for meals, placing napkin, flatware, condiments, straws and diet cards on trays.

12. Assist with tray service to residents in dining area and pick up dirty trays after meal is served.

13. Wipe dining tables following meals.

14. Assemble, prepare for washing, and wash dishes. Store clean dishes in proper place.

15. Assist with general cleaning and food preparation tasks as needed.

16. Maintain daily care of dishwasher and dishwashing area, lavatory and sink, work table, dish storage cabinet, and food carts.

17. Empty trash cans daily and clean them thoroughly. Replace liners and return cans to stations.

18. Assist with care of floors in kitchen by sweeping and/or mopping at a time when food is not being prepared or served.

19. Be aware of resident nutrition needs or restrictions and follow all care plans.

20. Ability to reasonably conform to all rules and policies of the facility.

Duties:

  • Assist with daily living activities. This includes delivering meals, turning and ambulating patients and providing plenty of fresh water, as well as extra nourishment between meals
  • Help patients with personal hygiene. Provide patients with bedpans, help with baths, shampoos and showers
  • Provide adjunct care for the patient, including ice packs, non-sterile dressings and therapeutic baths
  • Check vital signs and record daily information in the patient's chart
  • Assist the nurses and other staff as needed
  • Adhere to professional standards, follow policies and procedures and abide by federal, state and local requirements.

Requirements:

  • High School diploma or GED
  • Completion of a state-approved CNA certification training course
  • Basic computer skills
  • Ability to work as part of a team and to handle multiple tasks safely and effectively
    Good communication skills, including the ability to communicate sensitive information with empathy ("bedside manner")
  • High tolerance for working under pressure and handling critical situations with a cool head

Position: Director of Clinical Services Home Health

Job Type: Full-Time

Shift: Day Shift (Monday through Friday)

Compensation: $82,000-$92,000 annually

Work Location: Various- Lincoln, Omaha, Nebraska City, Council Bluffs, IA

Duties:

1. Participates in developing standards which ensure safe and therapeutically effective service to patients and families. Has joint responsibility with the Administrator for seeing that standards are met.

2. Participates in developing objectives for Agency.

3. Is responsible for seeing that objectives are implemented.

4. Consults with the Administrator to determine a staffing pattern which will accomplish stated objectives and promote maximum level of utilization of health personnel.

5. Is responsible for recruiting, hiring, evaluating and terminating service personnel.

6. Participates in planning for the orientation of new employees. Conducts selected orientation classes.

7.Plans and arranges for consultation needs of staff; prepares and maintains current policies and procedures which meet Medicare, Medicaid, accrediting bodies, state, etc. laws and implements such; revises concurrently.

8. Displays a willingness to support the policies and procedures and uses appropriate channels for change of such policies; establishes criteria and procedures for selection, promotion and termination of employment of service personnel.

9. Participates in establishing the functions and qualifications for each service position; coordinates interdisciplinary team services:

-Assures ongoing assessment of patient/family needs and implementation of interdisciplinary team plan of care.

- Assures physician approval of plans for continuity of medical care.

- Provides individual or group support concerning job related stress or issues.

10. Organizes the Agency to delineate and delegate authority, functional responsibility, lines of relationship and communication to provide safe and therapeutically effective service.

11. Participates in coordinating Agency's services with services of other community agencies.

12. Serves on utilization review committee.

13. Participates on QAPI.

14. Participates in studies and research and other administrative functions as assigned.

15. Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation and job performance and attendance.

16. Observes confidentiality and safeguards all patient related information.

17. Accepts responsibility for regular attendance and punctuality, fulfills job requirements without regard to time involved.

18. Serves as a resource person to employees.

19. Develops a cooperative relationship and communicates effectively and professionally with the physicians.

20. Investigates and reports any problem relating to patient care or conditions which might harm the patient and/or employee well-being.

21. Immediately reports any accident, incident, lost articles or unusual occurrence to the Administrator.

22. Attends pertinent continuing education programs, other than routine in services and shares information with staff.

23. Conducts at a minimum quarterly in person staff nurse meetings

24. Works with Administrator in identifying budgetary requirements and determining appropriate use of allocations.

25. Monitors equipment for appropriate use and take steps to keep misuse to a minimum.

26. Oversees Agency's ongoing performance improvement (PIP) program.

27. Responsible for overseeing development of indicators with appropriate data collection, aggregation and analysis, taking action and reporting results according to Agency's PI plan.

28. Provides 24 hour/day, seven (7) days/week on-call coverage.

29. Plans and supervises the Agency.

30. Ensures that patients’ plans of care are developed, implemented and evaluated.

31. Conducts monthly patient case conferences, in-services, staff meetings and maintains documentation; participates in community education projects.

32. Ensures that all necessary supplies and equipment are available.

33. Is normally available at all times during and after operating hours; may designate a qualified temporary replacement if he/she will not be available.

34. Ensures compliance with federal, state, accrediting bodies, local and Agency policies in all patient care aspects of the Agency.

35. Oversees staffing and visit assignments. If visits are not covered by current assigned staff, DCS will take responsibility to ensure visits are completed and patients do not go without care.

36. Addresses and corrects all patient concerns.

37. Will be responsible alongside Administrator for providing onsite leadership during agency licensure recertification visits, compliant visits, revisits

38. Will complete at a minimum quarterly in home staff nursing visits for audits and ensure ongoing compliance.

39. DCS will be responsible for documentation review and submission for the skilled nursing, LPN/RN and HHA.

40. DCS will run the weekly Skilled Nursing Case Conference Meeting and attend and participate in weekly Agency Operational Meeting alongside Administrator.

41. DCS will review all patient referrals through the referral portal(s) and ensure staff availability for accepting patient referrals in throughout each day.

42. Utilizing all forms of communication (OHMD app, email, text, call, Wellsky hotbox) with intake coordinator, interdisciplinary team, administrator, ect) in a timely basis. All communication should receive follow up by DCS by end of each business day.

43. Responsible for developing and implementing On-Call Staff Nursing Schedule.

44. Overseeing supplies availability at home office and reordering supplies as needed.

45. Development and implementation of annual nursing competencies ensuring compliance with agency and state policies.

46. Work alongside Administrator for the development of Emergency Preparedness Drills for compliance.

47. DCS will assign and/or ensure completion of in person home health aide supervisory visits every 2 weeks and biannually with home health aide present.

48. DCS will develop and implement infection control program.

49. DCS will review submitted weekly case conferences and 30 day summaries to ensure a detailed summary of care for each discipline of care has been displayed.

50. DSC will complete routine chart audits for patients addressing frequencies, visit completion, orders, documentation.

51. Evaluating weekly clinician schedules and rearrange schedules as necessary to avoid overtime.

52. DSC will track overdue documentation and will communicate with clinicians and develop a plan to complete outstanding work in a timely manner to remain compliant for billing purposes.
