Unit: Pediatric Unit (LTAC Hospital)

Shift: Day Shift (8 hour shifts. Monday through Friday)
Compensation: Based on Experience
Work Location: LTC Acute Hospital - In person at 1540 N 72nd Street Omaha, NE 68114
Duties:
- Provide emotional support to patients and their families
- Obtain social history
- Empathetic listening
- Psycho-social assessment
- Problem solving
- Insurance/Medicaid knowledge
- Case management
- Discharge planning
- Care plan assessments/MDS
- Working with school systems
- Family and multidisciplinary team involvement
Qualifications:
- Bachelor's Degree in Social Work from an accredited school of social work
- One year of medical social work experience preferred
- Social work License Required
