Nebraska City, NE - Ambassador Health of Nebraska City is excited to announce Teri Stukenholtz as its new social services director. Teri joined the Ambassador Health team in May, after working with the State of Nebraska Department of Health and Human Services for 21 years. During 16 of those years with DHHS, Teri worked with Nebraska Medicaid, specifically with disabled and elderly clients who were entering assisted living or long term care.
As the social services director, Teri is responsible for ensuring that the emotional and social needs of the residents are met and maintained in accordance with company policies and federal and state regulations. In addition, Teri completes psycho-social assessments and develops a comprehensive social history, identifies and addresses unmet resident needs and wants, assists residents with financial questions, participates in goal setting and care conferences, and coordinates resident discharge plans with community resources.
